Dashboard: Notification form UI updates

  • Move "Cancel" and "Add Notification" buttons for notifications form from the summary on the right-side to below the form on left-side. On smaller devices, the summary moves to the left-hand side form where it slots in between the form and the "Cancel" and "Add Notification" buttons.
  • Simplify notifications form input field labels.
  • Update Environment Type field selection to new design.
